You will need to turn these in before you are eligable to play in the games and tournaments
The purpose for this form is to enable parents or gaurdians to authorize the provision of emergency treatment for their children if they become injured or ill under the authority of the coaches, certified chaperones or faculty advisor Ben Zhao in the event that the parents or guardians cannot be contacted.

The TCUL Waver is only needed for the Fall season because we play through the Twin Cities Ultimate League
